Document quality checklist

Questions and instructions
- Each question names the information needed.
- Required and optional answers are clear.
- Examples are realistic and contain no real personal data.
- People know what to do when a question does not apply.
- Date, number and length requirements are explained before submission.
Checks and review
- Each check has a reason connected to the process.
- Related answers are checked together where needed.
- Uncertain answers can be reviewed by a person.
- Passing document checks is distinguished from final approval.
- A reviewer can inspect the original document.
Corrections and data handling
- Feedback identifies the answer to change and the required correction.
- People know how to resubmit or ask a question.
- The receiving organisation and privacy information are visible.
- Only necessary personal information is requested.
- Retention and access arrangements have been agreed.
