How to write useful document correction feedback
Feedback should help someone fix the document without another explanation from the reviewer. State what is missing or unclear, what is needed and where to change it.

Keep requirements and decisions distinct
Explain a failed document requirement without implying that the underlying business request has been refused. If a person needs to decide, say who will review it and what the sender should do.
Allow questions
Provide a contact at the receiving organisation for disputed requirements. A clear explanation and a person able to reconsider it help prevent repeated, unproductive corrections.
